After the tour, our staff will meet with you to discuss your loved one’s specific needs and how we can best meet them. If you’re ready to begin the admissions process at that time, we will guide you through the necessary paperwork and assessments to ensure a smooth transition.

  • Power of Attorney papers
  • Health Care Proxy
  • Medical Orders for Life-Sustaining Treatment (MOLST)
  • Living Will – Non Hospital DNR
  • Medicare and Health Insurance Cards
  • Immunization records if available
  • Any information regarding new resident’s social history, i.e. information regarding parent, siblings, education, employment, military service history

For Resident

  • At least 7-10 days of clothing (must be able to withstand high heat laundry)
  • Rubber soled shoes or sneakers
  • Underwear, socks, pajamas if desired
  • Cable ready television 19″ to 32″

Resident may bring

  • Twin-sized bedspread or comforter
  • Plants
  • Photographs
  • A few knick-knacks (items that will help provide a “home-like” atmosphere)
